United American offers a variety of Hospital and Surgical expense plans for individuals and families. These plans are designed to supplement current health coverage or serve as basic coverage where cost or availability is an issue.

Freedom Signature Series (GSP3, SWL, RT10)
> Issue ages 0-63
> Sliding scale of premiums and benefits
> Choice of deductibles and maximum benefit amounts
> Doctor Office Visit Benefit
> Miscellaneous Hospital Expense Benefit up to $50,000
> Optional cancer, critical illness, and accident benefit riders
> Choice of whole lilfe or term life policy on the same convenient application
 
Foundation Signature Series (MMGAP, SWL, RT10)
> Issue ages 0-63
> Helps manage your major medical deductible, copays, and coinsurance
> Pays benefits for hospital inpatient expenses
> Calendar-year maximum benefits up to $10,000
> Optional hospital outpatient benefit rider
> Choice of whole life or term life policy on the same convenient application
 
Tradition Signature Series (INDEM1, SWL, RT10) - California Only
> Issue ages 0-63
> Hospital inpatient benefit pays up to $50,000
> Mammography benefit for women
> Optional outpatient physician benefits, cancer, critical, illness, and accident benefit riders
> Choice of whole life or term life policy on the same convenient application
 
FLEXGUARD® Plus Hospital & Surgical (GSP2)
> Issue ages 0-63
> Coverage for individuals or families
> Choice of three benefit levels: $50,000, $75,000, or $100,000.
> Choice of additional benefits: Additional physician charges, critical illness benefit, cancer, or accident
 
Good Sense Plan Hospital & Surgical (GSP1)
> Issue ages 0-63
> Choice of daily hospital room, intensive care, and outpatient hospital expense
> Choice of surgical expense benefit
> Miscellaneous hospital expense benefit of up to $25,000
> Miscellaneous surgical expense benefit of up to $7,500
 
Common Sense Plan (CS1)
> Issue ages 0-63 (0-60 California and Texas)
> No lifetime maximum
> Maximum hospital/surgical benefit amount per covered family member, up to $100,000 per year
> Choice of annual deductible up to $5,000
 
HSXC Hospital & Surgical
> Issue ages 0-63
> Choice of daily hospital room benefit
> Miscellaneous hospital expense benefit up to $7,500
> Additional benefits for intensive care, surgery, assistant surgeon, and more
 
SHXC Hospital & Surgical
> Issue ages 0-63
> Choice of daily room benefit
> Includes intensive care, surgical, and miscellaneous hospital expense benefits
 
SMXC Surgical & Medical
> Issue ages 0-63 (0-60 California and Texas)
> Doctor treatment benefits
> Surgical expense, X-ray, laboratory benefits
 
MMXC Hospital & Surgical
> Issued at all ages (not available if enrolled in Medicare)
> Choice of daily room benefit
> Provides hospital room benefits for as long as two years
> Miscellaneous hospital expense benefits up to 20 times daily room benefit
> Additional benefits for intensive care, surgical expense, assistant surgeon, and more
 
HealthGuard Critical Illness (CILS)  
> Issue ages 18-64
> Choose a lump-sum benefit amount: $10,000; $20,000; $30,000; $40,000; or $50,000. (Benefit amount will reduce by 50% at age 65)
> Guaranteed Renewable to age 80
> First diagnosis coverage; policy terminates upon payment of benefit
> Pays in addition to any other life, major medical, or hospital coverage
> Individual or spouse coverage available
> No Precertification requirements
 
Cash Benefit Cancer Plan (CANLS2)
> Issue ages 0-69
> Choose a lump sum benefit amount: $10,000; $20,000; $30,000; $40,000; or $50,000. (Benefit amount will reduce by 50% at age 65)
> First diagnosis coverage; policy terminates upon payment of benefit
> No physical exam, just a few health questions determine eligiblity
 
UA-250 Accident Compensation Plan
> Issue ages 0-63
> Pays benefits for accidental injury
> Choice of lump-sum payment or hospital cash benefit
   

All United American policies are guaranteed renewable, which means your policy cannot be canceled for any reason as long as premiums are paid on time. Benefits cannot be changed by the Company because of any change in your health or the number of times the policy is used.
